The Terraces at Los Altos – a HumanGood community Looking for RN- Admission Nurse at Atherton, California Full Time

This is an incredible opportunity to work within a beautiful 30-bed, private bath Health Center. This position has primary responsibility for admissions and treatment functions.

Shift: 10 am – 6 pm (rotating 4-2 schedule)

Hourly: $50.00-$55.00

• Works with hospital case managers, the patient, and the patient’s family to provide placement for patients who require skilled nursing services or skilled rehab therapy.
• Communicates with the appropriate interdisciplinary team member to ensure a smooth transition for patient from the home/healthcare settings to HumanGood Skilled Nursing Health Center.
• Communicates the patient’s plan of care to interdisciplinary team members to ensure quality care.
• Monitors reviews and manages the inquiry process through the Referral Manager System as needed to assist the admissions team.
• Supports admission coordinator with inquiry calls from hospitals, assisted living, families and other healthcare facilities.
• Maintains communication and rapport with hospital accounts, ACOs and other referral sources.
• Ensures utilization of standard admission process via EHR system. Manages inbound Continuity of Care Documents (CCD) via Care quality EHR integration.
• Communicate the new admission’s specific needs to interdisciplinary team members and the primary care physician or other providers to ensure Person-Centered quality care is provided and to support discharge goals.

To be successful in the role, you would have:

• Current RN license (or obtain within 90 days) in the state for which applying
• Current CPR certification
• Prior nursing experience: senior care, skilled nurse, post-acute or sub-acute care preferred
• New grads are welcome!
