Job Description Summary
Coordinates the admission assessment process, leads referral development activities and coordinates the PPS process for the SRALAB alliance.
Coordinates referrals to the SRALAB Alliance including review of medical records for patients referred to determine rehabilitation potential and making recommendations for the most appropriate rehabilitation level of care.
Facilitates admission to the acute inpatient rehabilitation program for appropriate referrals through coordination with the Admitting Department at the alliance.
Establishes, develops, and maintains excellent referral relationships with referring physicians and discharge planners at the SRALAB alliance and targeted facilities.
Responsible for clinical data collection, data entry, transmission and analysis of IRF-PAI data and ensuring compliance with the requirements of the Medicare Prospective Payment System.
Additionally, responsible for maintaining expert knowledge of PPS rules and regulations and providing education, mentoring, and auditing services for the alliance.

Job Description
Coordinates Admissions to the SRALAB Alliance : Acts as a resource to all referring parties regarding available services at SRALAB and Continuum.
Establishes and maintains excellent relationships with all referring facilities, physicians, and associated clinical staff.
Serves as a representative of SRALAB to referring physicians and discharge planners in order to identify their needs for rehabilitation services, increase their satisfaction with SRALAB and therefore, improve referral relations.
Reviews medical records of patients referred to the program to determine rehabilitation potential, appropriate level of care, and patient’s nursing needs.
Confers with patient and family to determine their goals and expectations for rehabilitation and provide orientation as appropriate.
Assess medical stability and nursing needs of inpatient referrals through the intake channel and communications to the SRALAB physicians and associated clinical staff.
Maintains responsibility for coordination of special patient services on a pre-admission basis such as dialysis, chemotherapy, radiation, special equipment needs, nutritional needs, etc.
Leads referral development activities for the Alliance in an effort to establish and improve relations with referral sources (eg.
off-site marketing presentations, meetings, luncheons, breakfast meetings, group and individual tours, etc.).Provides timely and appropriate completion of all pre-admission documentation including documenting the Preadmission Assessment in the medical record, with prompt and complete communication to nursing units.
Negotiates any conflicts arising from the referral of a patient’s care and ensures a holistic approach to problem solving in order to provide a smooth and efficient transfer process.
Serves as a clinical liaison to the Nurse Manager regarding daily patient admissions / transfer / discharge activity.Acts a resource to Third Party Payors and provides findings from a patient on-site evaluation as requested.
Includes pre-admission development of patient goals for rehabilitation and negotiation and pre-certification of initial approved length of stay.
Maintains excellent communication and relations with other departments including Patient Financial Services, Pharmacy, Central Supply, Scheduling, Medical Staff Office, Bed Control, Care Management, etc.
Conducts tours of the Institute for families, referrers, potential referrers, and managed care representative. Encourages development of professional relationships with Institute nursing staff, Institute clinicians.
Provides ongoing distribution of Public Relations materials to all potential referral sources.Collects and trends referral and admission data and reports data monthly to the medical and administrative leadership at the alliance and at SRALAB.
Perform all other duties that may be assigned in the best interest of SRALAB.Administrative ResponsibilitiesCoordinates the PPS Process at the SRALAB AllianceCollects required data from inpatient medical records to complete the IRF-PAI.
Works with clinical staff to ensure accurate and timely data collection and transmission.Analyzes data to ensure completeness and consistency prior to transmission.
Remains abreast of changes to the form / process and regulation for data collection.Collaborates with Information Systems, Medical Records and Patient Financial Services as needed.
Performs clinical assessment of patient as needed.Communicates regularly with clinical and administrative staff to promptly collect missing data or to research data that appears inaccurate or inconsistent.
Completes data entry as needed.Plans and coordinates educational sessions for clinical staff at the alliance including orienting new staff and documenting competency.
Participates in committees and on projects associated with PPS.Reporting RelationshipsPrimary reporting relationship is to the Executive Director for the alliance.
Knowledge, Skills & Abilities RequiredMust be a graduate of an approved program resulting in an RN or BSN degree.Must possess current license as a Registered Nurse as required by the State of Illinois.
At least one year of rehabilitation nursing experience in order to display a full knowledge of rehabilitation and health care delivery systems.
Must exhibit excellent knowledge of the organization, display confidence in patient assessment skills and communicate thorough understanding of rehabilitation medicine and its goals as related to patient care and outcomes.
Excellent interpersonal skills necessary to interact effectively with physicians, clinicians, insurance representative, patients / families, and a wide variety of potential referrers.
These skills are necessary to effectively represent within the community.Analytical skills necessary to develop plans, goals, and procedures in order to solve complex administrative and referrer / patient problems as well as maintaining a key sensitivity to payor considerations.
Advanced reasoning and problem solving skills required for data analysis of referral data and PPS data.Working ConditionsWorks routinely in patient care and office environments where there are minimal physical discomforts due to temperature, noise dust, and the like.
Occasional exposure to infectious disease when working with patients.Maybe necessary to travel to other health care facilities.
