This individual will be expected to monitor patients’ aftercare and support them in achieving short / long term health goals.
The medical assistant will coordinate with providers to ensure patients’ needs are met. A working knowledge of the clinical field is desired for this position.
Care Manager Responsibilities : Manage a roster of patients who have enrolled in the remote patient monitoring program.Have a full understanding of each patients individual care plan.
Record patient data daily and alert healthcare providers of abnormal reports.Care Manager Qualifications : At least 1 year of clinical work experience as a Medical AssistantAt least 1 year of EMR experience;
