GLIDE Hiring for Harm Reduction Case Manager at San Francisco, CA, United States Full Time

GLIDE’s Harm Reduction (HRx) programs are at the forefront of innovative and collaborative approaches in San Francisco. The program’s goals are to improve our community’s wellness through linkages to medical services, collaboration with community and city providers, and advocacy for underserved and marginalized individuals.

HRx programs includes Syringe Access Services (SAS), HIV / Hep-C / STI Testing and System Navigation, Law Enforcement Assisted Diversion (LEAD), Project OPT-IN, street outreach, and overdose prevention training.

The (SIP) Harm Reduction Case Manager provides individuals with a history of substance use and co-occurring disorders who are in immediate need of housing, basic items, food, health services, and other support due to homelessness and health risks for COVID-19.

The (SIP) Harm Reduction Case Manager assesses clients’ immediate needs and provides information and support to overcome barriers, provide early-interventions, and link to long-term services and care.

Services are provided in person and virtually at various community-based settings within the Tenderloin and SOMA. Regular and predictable attendance is essential.

Responsibilities and Duties :

• Provide low-threshold advocacy and case management, which includes linkages, referrals, and assistance to housing, health services, legal aid, and other human services and resources.
• Conduct outreach, needs assessment, service planning, service coordination, and follow up contacts in person or by phone with clients and other providers.
• Develop rapport and trust with clients, through motivational interviewing techniques, to ensure a smooth and successful transition to long-term services and their providers.
• Develop and maintain an effective network, based on collaborative efforts, with other GLIDE service providers and appropriate community service agencies, to provide comprehensive services to clients.
• Document service contacts and other efforts program database and maintain accurate, current data, records and files, and documentation of clients’ progress.
• Apply strength-based, client-centered approach with adherence to cultural humility and trauma-informed service and harm reduction principles.
• Attend weekly individual supervision and other administrative meetings as directed by supervisor.
• Adhere to professional boundaries and standards, including requirements of HIPAA, best practices to maintain confidentiality, and reporting requirements (e.

g., child reporting, adult abuse, etc.).

Meet professional obligations through efficient work habits such as : collaborating with co-workers, meeting deadlines, honoring schedules, attending, and actively participating in staff and individual meetings, supervision, case conferences, client conferences and in-service trainings.

Qualifications :

• Direct service experience with harm reduction services (syringe access, outreach, Narcan trainings, and health navigation) desired.
• Demonstrated knowledge of conducting a needs assessment, collaborative approach to service planning, and basics of Motivational Interviewing.
• Experience working with individuals that have faced chronic / long-term homelessness, severe or persistent mental illness, substance use disorder, chronic / complex trauma, and of marginalized communities (based on race, gender, sexual orientation, etc.).
• Experience with crisis management and ability to provide de-escalation in individual and group settings.
• Knowledge of strength-based and client-centered approaches as well as trauma-informed and harm reduction principles.
• Knowledge of human services, healthcare, and other resources in San Francisco and greater Bay Area and experience navigating processes for application and enrollment.
• Understanding of systemic racism and other forms of discrimination as barriers to health, well-being, and self-sufficiency.
• Ability to plan, prioritize and organize tasks and meet deadlines with strong attention to detail, particularly in documenting case files and updating the database.
• Understands and follows basic ethics, values, and principles of case management regulatory practices.
• Excellent interpersonal and communication skills to establish and maintain effective working relationships with those contacted in the course of work.
• Works well under pressure and manages stress effectively in a demanding and fast paced environment.
• Demonstrates cultural humility and has the ability to work effectively across cultures.

Education and Experience Requirements :

• 1+ years harm reduction services, case management, navigation, or advocacy experience working with socially and / or economically marginalized, multiply diagnosed populations.
• Knowledge of violence prevention and intervention techniques. Experience providing direct social services to homeless and / or very low-income clients who are dual diagnosed and / or have experienced domestic violence, and / or have been incarcerated.
• Knowledge of community resources for assisting clients.
• Demonstrated understanding of lesbian, bisexual, transgender, and African American populations. Demonstrated understanding of the psycho-social and economic issues that may impact clients.

Certificates, Licenses, Registrations :

• Current California Driving License; and clean driving record.
• Alcohol and Substance Abuse certification and / or Harm Reduction Counselor certification desired; (can provide training if needed).

$25.45 – $29.42 an hour
