DEFINITION:

A registered nurse providing professional leadership in the delivery of nursing services necessary to support, maintain and implement the home care program in an assigned area, based on client need

SUPERVISOR:    Director of Clinical Services

GOAL:

The delivery of professional, reliable, competent and compassionate home health services to people who have complex health care challenges

CLASSIFICATION:

Non-exempt

ESSENTIAL RESPONSIBILITIES AND FUNCTIONS:

Continuous skilled nursing services include:

• Activities of daily living assistance,
• Breathing assistance, extraordinary (mechanical ventilation, oxygen delivery, respiratory treatment, tracheostomy tube) and ordinary,
• Hydration and nutrition assistance, extraordinary (central line, gastronomy button, gastronomy tube, nasogastric tube) and ordinary,
• Instrumental activities of daily living assistance,
• Medication administration assistance, and
• Toileting assistance, extraordinary (catheter tube, ostomy bag) and ordinary

QUALIFICATIONS:

   Graduate of an accredited nursing program as a registered nurse as required by the Massachusetts Board of Nursing Registration

    Licensed currently to practice as a registered nurse in the Commonwealth of Massachusetts

   Maintenance of optimal nursing practice through continuous professional growth and development

  One year of nursing experience as a practicing nurse is preferred

AGENCY REQUIREMENTS:

   Access to transportation

   Valid state driver’s license

PHYSICAL REQUIREMENTS:

     Must be able to stand/walk a minimum of hours per -hour working day

     Must be physically capable of providing physical care to patients who are dependent in all aspects of their care

     Frequent bending to level of standard bed, wheelchair, floor, commode, shower chair, bath tub and performing procedures such as dressing changes, catheterizations, CPT, transfers, nursing interventions, ADL’s and home exercise programs

     Ability to reach and lift above the shoulder level as needed to perform all procedures needed for patient care in the home care setting

     Occasional transfer and/or lifting of pound patient without assistance and > pound patients with assistance

     Frequent entering/exiting of automobiles with driving average of miles/day

     Must be able to remain awake and alert at all times

     Must be capable of arriving to all assigned shifts no later than the scheduled time

     Ability to function effectively in noisy and busy environments
