Follow and adhere to care and service plans as instructed Instruct, supervise and evaluate in all areas of community living.
Implement all behavior plan interventions as assigned.

Maintain valid Driver’s License and provide transportation as required by program.
Assist individuals in scheduling and attending medical appointment.

Communicate daily activities, progress, and service provision information to family members.
Actively assist and support as needed with personal care responsibilities, may include:
diet, exercise, meal preparation, bathing, shaving, dressing, toileting etc.

Assist and support as needed with physical direct care, may include:
physical or mechanical transfer of individuals from wheelchairs to van, chair, or shower, etc.
Administer medications if necessary as required, in accordance with medication administration procedure.

Attend any required meetings
Qualifications:
High School Diploma or equivalent.
Valid Driver’s License.

Basic knowledge of Microsoft Word and the internet.
