Job Summary:
The Help at Home Health Equity Program Manager will lead the development and execution of our Healthy Community Program. This is a leadership role that requires a high degree of collaboration and project management skills.
Responsibilities:
Execute our aim to address health disparities for our clients.
Manage the day-to-day operations of the program
Lead, direct and coach the performance of community health workers
Supervise teams of community health workers
Hire, develop and retain staff; oversee onboarding, professional development, and continuing education for community health workers
Supervise and assist program staff to execute deliverables, while establishing performance goals, ensuring progress towards milestones, and providing coaching and feedback.
Build relationships with health care service delivery partners and community-based organizations. Lead the curation of a library of community-based resources to address clients’ SDOH.
Curate and maintain operational and outreach materials used by Community Healthcare Workers.
Drive client and caregiver NPS as it relates to the program.
Collaborate and communicate with a wide array of staff and stakeholders
Present program data to internal and external stakeholders
Conduct on-going assessment of program outcomes to determine the effectiveness of the program
Lead and sustain program integrity and efficacy through continuous quality improvement
E nsure that program goals are met, measured, analyzed, evaluated, and reported
Qualifications:
3+ years of experience as a Program Manager
Master’s degree in public health, Master of Social Work, or related field
5+ years of experience working in community-based organizations serving culturally diverse, very low-income populations
Results-driven, experienced health care professional
Demonstrated knowledge related to program development
Proven stakeholder management skills
Demonstrated knowledge and experience promoting health equity
Demonstrated understanding of social determinants of health
Knowledge and Skills:
Passion for advocacy, advancing health equity and social justice
Strong analytical skills
Proven ability to solve problems creatively
Ability to work independently and as part of a broader Care Coordination team
Excellent interpersonal, written, and verbal communication skills
Organized, flexible and able to multitask
Ability to manage ambiguity and work within a rapidly changing environment
Advanced knowledge of Microsoft Office products
