Job Summary
Home Hospice services from the Good Samaritan Society provide people of all ages medical assistance in their homes, while offering support and respite for their loved ones. The Weekend Hospice RN is a key position in working with the home hospice team. The right candidate is a team player and can prioritize effectively to accomplish the needs of the agency. Job Description
Establishes a service plan for each patient and evaluates on an on-going basis. Ability to communicate with patients, family members and others on the health care team.
Coordinates with interdisciplinary team as needed.
Completes required documentation in appropriate timeframes. Explains advance directives; clinical requirements and interdisciplinary services available through agency.
Supports staff in a hospice home environment within a 60-mile radius per Medicare guidelines.
Works independently with minimal supervision and seeks guidance as needed.
Performs initial patient assessment and ongoing assessments of the patient s status.
Participates in creation of the plan of care. Ensures the plan of care is implemented by the interdisciplinary group.
Processes physician services/orders.
Completes quality assurance audits as assigned.
Participates in agency in services; educational opportunities; team meetings and committees
Defines the unit of care (family and patient). Demonstrates knowledge of hospice palliative care, hospice philosophy, and scope of services.
Recognizes different cultural attitudes and values towards death and dying. Identifies the five stages of grieving process.
Explains advanced directives, clinical requirements, and interdisciplinary services available.
Utilizes independent nursing judgment when integrating health care.
Must demonstrate independence, assertiveness and critical thinking when working with patients and co-workers.
Ability to document clearly, solve problems and be self-motivated. Works with little direction. Must be able to establish priorities, have strong flexibility and organizational skills.
Assists with customer experience/marketing activities to expand community reach.
Assesses and formulates a plan of care for patients in their homes that include social, emotional, and physical needs.
Provides nursing care to patients according to physician orders and regulations. Assures physicians orders are processed and physicians are notified in a timely manner.
Qualifications
Graduate of an accredited school of nursing.
Licensure for state of practice is required in Nursing or related field preferred.
Must be willing to rotate weekend night call with staff to facilitate adequate coverage for all patients.
Prior work experience in like or related position may be helpful.
Ability to meet the physical and mental abilities of the job.
Must be able to speak, read and write in English and be able to articulate clearly and audibly.
Must have a favorable motor vehicle record (MVR) and must present a valid driver s license and proof of insurance before operating a vehicle.
Current CPR certification or ability to become certified in CPR within 90 days of hire.
Must be at least 18 years of age.
Must have or be able to obtain an Arizona Fingerprint Clearance Card
Qualifications
Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor’s degree in nursing preferred.
Minimum of one year of professional nursing experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required. Basic Life Support (BLS) required. Certificate for Hospice and Palliative Nurse (CHPN) preferred.
